La creatinina, es un producto final del metabolismo muscular, eliminada del organismo básicamente por filtración glomerular. Sus concentraciones séricas tienen una relación inversa e hiperbólica con la tasa de filtración glomerular, que es considerada el mejor marcador de función renal. Así, el método más utilizado para calcular la tasa de filtración glomerular estimada (TFGe) son las ecuaciones que la predicen a partir de las concentraciones séricas de creatinina. Las mediciones de creatinina sérica en la práctica clínica se suelen hacer por reación de Jaffé con picrato alcalino (por método cinético o punto final) o por método enzimático.

Los métodos de referencia como las cromatografías gaseosa y líquida acopladas a espectrofotometría de masa por dilución isotópica (IDMS por sus siglas en inglés) no son de uso rutinario. En 2002 aparecen las guías K/DOQI que unifican los criterios para el diagnóstico y clasificación de la ERC, en base a la (TFGe) y la presencia de marcadores de daño renal. En este documento también se recomienda que los laboratorios informaran la TFGe y se categorizaran los sujetos por estadios de ERC según la clasificación recomendada.

En la medida que esto se fue implementado se hizo evidente la gran variabilidad de la TFGe de los pacientes atendiendo al método de mensuración utilizado para la creatinina sérica. Como consecuencia de ello se desarrolla un documento de consenso del Programa Nacional de Educación Renal de EE.UU. (NKDEP) y la Federación Internacional de Química Clínica y Medicina de Laboratorio (IFCC) en 2006 en el que recomiendan recalibrar los métodos de mensuración de creatinina sérica para que sean trazables a un método de referencia (IDMS) o utilizar métodos enzimáticos, y minimizar así el sesgo entre métodos y laboratorios.

La mayoría de las mensuraciones de creatinina se hacen por método cinético de Jaffé (picrato alcalino), ya sea de forma automatizada o manual, sin corrección IDMS. El método enzimático, que resulta 10 veces más caro, tiene una disponibilidad muy limitada. Además, los laboratorios no informan rutinariamente la TFGe y la clasificación de los pacientes por estadio de ERC. Ante esta situación se han desarrollado ecuaciones que permiten corregir las concentraciones séricas de creatinina mensuradas por método cinético de Jaffé, ya sea de forma automatizada o manual, contra un método IDMS trazable, lo que se le conoce como método cinético de Jaffé compensado.

Sin embargo, no se cuenta con una herramienta que permita de forma automatizada corregir la creatinina sérica según el método de medición empleado, y hacer los cálculos de la TFGe por las ecuaciones recomendadas por las guías internacionales, así como clasificar a los pacientes por estadios de ERC. Como respuesta a este problema se desarrolla este trabajo con el objetivo de presentar una aplicación informática que corrige la creatinina sérica a un método IDMS trazable (Jaffé compensado), calcula la TFGe según las preferencias del usuario (dentro de las ecuaciones recomendadas) y clasifica al sujeto por estadio de ERC, de forma automatizada.
